Description

Position Summary The Intake Coordinator function is to facilitate the referral intake process, coordinate care with the interdisciplinary group, the patient, their family, and the referral source. As a Intake Referral Coordinator, you will: * Receives and appropriately documents new referrals in a timely manner. * Verifies each patient’s payor source and maintains and tracks insurance authorizations and initial certifications. * Participates in coordinating care with Nursing Supervisors, patients, and the interdisciplinary group. * Provides effective communication to patients, staff members, other health care professionals and referral sources. * Responds to Agency and patient needs in a professional and creative manner. * Demonstrates commitment, professional growth and competency. * Participates in Agency-sponsored in-service training. * Participates in the Agency’s QAPI program. * Promotes the Agency philosophy and administrative policies to ensure quality of care. Requirements * Minimum one year of experience in referral intake is preferred * Experience with third party payers, Medicare and Medicaid highly desirable * Medical Field background preferred, including familiarity with medical terminology * Excellent computer and customer service skills * LVN/LPN preferred * Valid Driver’s License * Valid Auto Insurance * HomeCare HomeBase experience preferred Benefits and Perks * Medical, Dental, Vision Insurance * 401k You’re eligible after 3 months of service/The plan is 100% fully vested immediately/Choice Health At Home contributes 100% of the first 3% you contribute each pay period * Health Savings Account * Life Insurance * Short & Long Term Disability Insurance * Paid Time Off and Paid Holidays
