Job Title: Intake Coordinator Job Type: Full-Time Benefits: Medical, Dental, Vision, Accident, Critical Illness, Pet plans available Disability plans available Employer paid Life Insurance HSA and FSA plans Mileage reimbursement 160 hours of PTO annually plus 9 paid Holidays Job Summary: The primary function of the hospice intake coordinator is to facilitate the referral intake process, coordinate care with the interdisciplinary team, the patient, their family, and the referral source. Responsibilities: Receives and appropriately documents new referrals in a timely manner. Participates in coordinating care with Nursing Supervisors, patients, and the Interdisciplinary Group/ Team. Provides effective communication to patients, staff members, other health care professionals, and referral sources. Responds to the Hospice’s and patient needs in a professional and creative manner. Demonstrates commitment, professional growth, and competency. Participates in the Hospice’s sponsored in-service trainings. Participates in the Hospice’s QAPI program. Promotes the Hospice’s philosophy and administrative policies to ensure quality of care. Requirements Experience: Hospice/Home Health experience preferred. High school diploma or equivalent. Skills: Excellent organizational and time-management skills. Strong communication and interpersonal skills. Proficiency in scheduling software and electronic health records (EHR) systems. Ability to work independently and as part of a team. Compassionate and empathetic demeanor towards patients and families. Transportation: Reliable transportation and valid auto liability insurance.
