We are looking to hire an Intake Coordinator to join our Hayesville, NC home health team. This is Monday through Friday, on-site position at our Hayesville office. Receives clinical information on new patient referrals to the home health agency Collects all data pertinent to the request for admission to include demographic data, medical history, medication, physical, discharge summary, labs, physician orders and insurance information Verifies insurance/payer’s source via DDE or specific system Ensures referral acceptance and communication within 15 minutes of receiving referral Notifies clinical designee responsible for scheduling and authorizations Contact patient/clinical designee prior to admission to ensure service location Liaisons between field, medical, office and referral source staff Obtains authorization from insurance companies and ensures physician licensure compliance Conducts follow up on current active patient hospitalizations and communicate with clinical supervisor to obtain orders as needed Maintains non-admit log Requirements High school diploma or GED equivalent Prior clinical or non-clinical home health experience required Knowledge of Medicare, TennCare, state and federal regulations, and accreditation standards as they relate to clinical home health a plus Proficient in Microsoft Office Suite including Outlook, Work and Excel Excellent verbal, written, organizational, collaboration, multi-tasking and time-management skills Strong initiative, follow through, process orientation, attention-to-detail Demonstrated confidentiality, professional and business acumen
