Work in an environment that offers:  Support for career progression and development An opportunity to work with a leading regional health service A strong attitude to teaching and learning   The Position Job Reference: 3058 Position Title:  Intake Nurse - TCP/GEM@Home Classification: ZF4 - ZJ4Salary per hour (exc. super): $59.09 - $62.20Hours per fortnight: 64Employment Status: Ongoing Part Time   Closing date: 12/10/2025    The TCP/GEM@Home Intake Nurse takes a lead role in the assessment, risk identification, care planning and ongoing management of clients admitted to  TCP and GEM@Home/RITH, and works collaboratively with direct care staff and care coordinators as part of a multidisciplinary team to meet client goals and achieve a timely and sustainable transition to the discharge destination. The Intake Nurse will develop collaborative relationships across the health service and community programs to provide high quality coordination, support and timely management of TCP and GEM@Home/RITH clients. The Intake Nurse acts as the central point of contact for all involved in the care of the client and will utilise strong communication skills to liaise with the multi-disciplinary team and clients/families.  Working with Bendigo Health Bendigo Health are a leader in regional health care, combining outstanding services with a strong education, training and personal development culture.
