Responsibilities

As an Intake Officer, you’ll be the primary point of contact for clients, carers and health professionals accessing our community health services. You’ll gather essential referral information, determine eligibility for services, and ensure accurate, timely input to support clinicians in providing the right care at the right time.

  • Provide timely, professional, client-focused service and referral support.
  • Manage demographic and clinical information with accuracy and confidentiality.
  • Utilise Microsoft Office, iPM, CERNER, My Aged Care, and internal systems for seamless data entry and workflow.
  • Proactively maintain updated knowledge of referral pathways and community services.
  • Contribute to a supportive team culture, working across a 7-day rotating roster.
