Main Street Project (MSP) is a community health Centre with a mandate to address the social determinants of health which give rise to issues of addiction, physical and mental health issues and homelessness. JOB SUMMARY

The Intake Worker is a key connection to potential and current participants of the Main Street Project’s Withdrawal Management Program. The Intake Worker will connect individuals attempting to access MSP services and assist with identifying bed dates, building a connection, provide access to resources, and set a participant up for success within our WMS programs.
The Intake Worker will work with WMS Case Managers to assist with connecting to primary care, income, addictions & mental health supports to break down barriers and advocate for their participants. Following the completion of a participants stay in WMS services, the Intake Worker will continue to make contact with discharged participants to maintain connection and provide outgoing support. Relationship building is the key function of this role.

QUALIFICATIONS

• Experience working frontline with people who are unsheltered, lived experience and/or a combination of education and experience.
• Knowledge of case management principles and processes – eligibility, assessment, planning, implementation, evaluating, transitioning, documentation and file management
• Experience creating and maintaining case plans, progress notes and other necessary documentation
• Experience working with Indigenous organization and/or communities
• Lived Experience: Knowledge gained from personal experiences relevant to the role.
• Experience in working with culturally diverse populations
• Computer proficiency in Microsoft Office and any other database software(s) used to track service delivery
• Non-violent crisis intervention (NVCI), suicide intervention, and/or trauma informed training considered an asset
• Current CPR, Basic First Aid, and Mental Health First Aid (MHFA) certifications.

KNOWLEDGE, SKILLS, AND ABILITIES

• Ability to apply to harm reduction approach and housing first principles to day-to-day work
• An understanding of and ability to work within the frameworks of client-centred care, self-determination, strength-based approach, and trauma informed care
• Excellent interpersonal skills and the ability to form relationships with community members including a proven ability to communicate appropriate professional boundaries
• Motivational interviewing skills
• Excellent verbal and written communication skills
• Knowledge of Indigenous History and Colonial History and it’s impact on the reality of Indigenous people in Winnipeg.
• Knowledge of TRC Calls to Action, MMIWG Call to Justice, and UNDRIP
• Indigenous language skills are considered an asset
• Knowledge of community resources available throughout Manitoba
• Knowledge of addictions, mental health diagnoses, and co-occurring disorders
• Knowledge and ability to adhere to confidentiality standards and legislation
• Ability to manage, assess, and work with challenging behaviours
• Ability to work independently, under pressure, in a team or independent setting
• Ability to effectively resolve and cope with conflict and/or crisis situations
• Strong collaboration skills, both with team members and with external partners

GENERAL INTAKE WORKER DUTIES:

• Bed Intake Monitoring: Monitor bed availability and intakes at two locations daily, ensuring timely and appropriate placement for individuals seeking withdrawal management services
• Client Outreach: Routinely connect with potential intakes to assess their needs, provide information about the program, and offer support throughout the admission process
• Post-Treatment Follow Up: Conduct post-treatment follow-up with Participants to assess their progress, address any concerns, and provide additional resources as needed to support their recovery journey
• Assistance with Transports: Facilitate transportation arrangements for Participants as required, coordinating with relevant parties to ensure safe and smooth transitions to and from the treatment facilities
• Collaborative Treatment Planning: Work closely with other WMS Case Workers to develop personalized treatment plans for participants; all unique needs and circumstances should be considered and reviewed
• Support the connection and work of Indigenous Relations and WMS Skabes within each program area and encourage participation in cultural programming through a lens of Healing and Harm Reduction
• Create and maintain a professional relationship with Community Members, including boundary setting, clear and concise messaging, and up to date documentation detailing interactions
• Keep accurate, continuous, legible and signed records including progress notes, checklists, correspondence, journals for individual and collective access in accordance with established protocols pertaining to access to and disclosure of WMS operational files
• Work with continuous care and consideration always taking the power differential between the Intake Worker and participant into account
• Planning process and intervention with Participants incoming and outgoing who have established connection with MSP
• Identify and provide information and resources to Participants including:
• Community Resources
• WMS Resources

• Mental Health Resources

• Indigenous Organizations

• Identify symptoms of suicide ideation and offer resources and support

• Create environments of cultural safety for participants
• Monitor, evaluate, and communicate progress of participants with the participants themselves, MSP WMS Staff and Manager/Coordinator/Director of Healing and Harm Reduction on a regular basis
• Maintain accurate and confidential electronic and file records, including statistical data
• Conduct Crisis Intervention as needed
• Attend program-related community, coalition, and committee meetings
