Description:

The role of the Integrated Care Home Co-ordinator is to deliver high quality, caring and compassionate patient care. The post holder will undertake assessment, implementation, and evaluation of patient care needs for a defined group of patients under the indirect supervision of a registered professional in their specialist area. The post holder is required to work as part of a team committed to delivering and improving a high standard of nursing care within UHCW.
Take a lead role in navigating referrals from healthcare professionals to the appropriate service, working autonomously and prioritising workload according to the demands of the service.
Work collaboratively with multidisciplinary teams and agencies, which may include health and social care services to support navigation of patient referrals into services which best meet their needs.
Will support patient flow out of hospital and through relevant Community Services
To assist managers and clinicians with service development and project work, data capture, audit, and reporting.
Will assist in facilitation of virtual MDT reviews including care home ward rounds
Full valid UK driving license and access to a car is essential for this post
Ensure records and documentation are maintained, kept confidential, and meets appropriate Trust standards, and in line with the Data Protection Act
To Support Operational Lead in development and delivery of service strategy and organisational development.
To form collaborative and professional relationship with relationships with the wider multidisciplinary team.
Demonstrates a positive and enthusiastic approach to their work in order to maintain high morale and motivation within the department. Promoting a professional and positive working environment
To work collaboratively with health, social care, voluntary or private sector managing referrals, enabling the provision of care from a variety of agencies. The post holder will provide central liaison between the services and the clinicians who refer patients
Demonstrate their own activities on new or less experienced employee
