Show effective communication skills, maintaining confidentiality at all times. Ensure patients needs are assessed and ongoing care is effectively implemented. Maintain accurate records and documentation both written & electronic. Take responsibility of own learning maintaining mandatory training, appraisal & professional registration requirements.
Active participation in improving other professionals understanding & learning regarding patient flow and discharge planning. Comply with CCICP policies & procedures, including responding to safeguarding concerns appropriately. Deputise for the leadership team when needed, demonstrating effective clinical leadership, organising other team members. Provide pastoral support to junior members of the team, including the expanding GNA service
