INTENSIVE CARE COORDINATION PEER SUPPORT SUCCESS PROFILE - CLACKAMAS COUNTY

As a Peer Support Specialist on FolkTime’s Intensive Care Coordination (ICC) Team, you will integrate into a team of Clackamas County mental health professionals to support county community members who are navigating complex, overlapping care needs and service providers. Employing the Intentional Peer Support model, you will prioritize advocating for peers during care coordination meetings, connecting people with community resources, trust-building and support navigating complicated care situations. Responsibilities include guiding peers as they navigate complex care requirements, enhancing community support networks, and ensuring the peer is heard during coordination meetings.
You will provide compassionate, personalized support, connect peers with essential services, and ensure they feel safe and supported. Through your calm and presence, you will make a lasting impact on the lives of Clackamas community members.
