Position Title: Intensive Case Manager (Part-Time)
Reports To: Operations Manager
Employment Term: November 1 – March 31, 2025
Hours: Part-Time (Flexible scheduling, 20 hours/week)
Compensation: Up to $27.54 per hour (depending on experience)
Position Summary:The Intensive Case Manager provides individualized support to winter shelter guests in areas such as housing, personal goal setting, mental health, and substance use. This role collaborates with shelter staff to identify guest needs, connect them with relevant resources, and support progress toward achieving stability and independence.

QUALIFICATIONS:

  • Diploma or degree in Social Work, Human Services, or related field (or equivalent experience).
  • Experience in case management, housing navigation, or mental health/substance use support.
  • Strong crisis intervention, problem-solving, and advocacy skills.
  • Knowledge of local housing and support services is an asset.
  • Criminal Record Check with Vulnerable Sector clearance required.

Responsibilities
  • Conduct guest assessments and develop individualized service plans.
  • Support guests with housing applications, referrals, and navigation of housing services.
  • Provide guidance and referrals related to mental health, substance use, employment, and personal development goals.
  • Build trusting relationships with guests while maintaining professional boundaries.
  • Collaborate with shelter staff, community agencies, and health professionals to coordinate services.
  • Maintain accurate case notes, documentation, and reporting.
  • Advocate for the needs of guests within community systems and services.
  • Attend Wellness Team Meetings weekly.
  • Attend weekly leadership meetings and monthly staff meetings.
  • Maintain confidentiality and adhere to shelter policies and procedures.
