Status: Full Time Hourly                                            FLSA Classification:   Non-Exempt Summary of the Position:  Intensive Family Coordinator provides a professional level of case management services as well as direct advocacy services to a caseload of families. * Intensive Family Coordinator will be responsible for completing family assessments, recruiting foster parents, completing home studies for the referring authority, locating family resources, and creating individualized service plans. * Case management services including intakes, safety planning, team meetings, ISP development 24/7 support, case contacts, and case record documentation in keeping with state regulations. Qualifications/Requirements: * High School Diploma or GED is required. * Experience with community work and at least one (1)  year experience working in human services field. * Experience in the provision of home and community based services. * Proficient in Microsoft Office Suite. Experience using an electronic health record (EHR) system is a plus. * Strong communication and organizational skills  * Must have reliable transportation, valid driver's license, and current auto insurance coverage. * Bilingual (Spanish Speaking) is a plus.
