MAIN DUTIES OF THE ROLE

• Provide expert clinical assessment, treatment, and support for individuals with dementia or suspected dementia in acute hospitals, care homes, and community settings.
• Deliver specialist advice to patients, carers, and professionals to promote high-quality, personalised care.
• Undertake bio-psychosocial assessments and contribute to care planning.
• Offer clinical guidance across health and social care teams, including legal framework expertise.
• Collaborate with multidisciplinary teams to develop and implement effective care pathways.
• Lead service development initiatives and contribute to research, audit, and clinical governance.
• Ensure services meet performance, quality, and information targets.
• Provide clear leadership and operational management of the liaison team, ensuring high standards of care delivery.
