Description

This position is accountable for those pharmacy services which require pharmaceutical education and state licensure; leadership experience and extensive clinical and professional judgment to provide quality care to Med/Surg patients. This vital resource works to establish clinical practices consisting of drug reviews for appropriateness, drug monitoring, participating in trauma codes and active involvement in the management of Med/Surg patients. These duties are performed according to departmental policies and procedures, and as scheduled by the supervisory staff. * Possesses excellent mathematical ability, attention to detail, concentration, and alertness. * Demonstrates the ability to organize and prioritize work with strong time-management skills * Safely handles medications in accordance with state law and hospital policy * Exhibits professional and respectable oral communication with co-workers, patients, outside vendors, and other health care affiliates. * Demonstrates the ability to multitask, work independently, and function effectively within a team * Can proficiently read, write, speak, and understand English. * Is open to receiving training, instruction, and ongoing professional development. Is open to leadership roles within the pharmacy residency programs. * Facilitates patient care through clinical activities including disease state management, pharmacotherapeutic monitoring, and pharmacokinetic monitoring * Serves as a pharmacotherapy resource to medical staff during multidisciplinary rounds and provides evidence-based drug therapy recommendations * Provides drug information services to physicians, nurses, and allied health professionals regarding dosing, drug interactions, adverse drug reactions, and medication use in special populations * Researches and develops pharmacy protocols and training procedures related to Internal Medicine patient care * Provides formal accredited educational sessions, including medical grand rounds and pharmacy education programs * Performs direct patient care activities, including bedside patient education when appropriate * Performs and documents drug therapy interventions in accordance with hospital formulary, protocols, and targeted medication programs * Investigates, documents, and reports adverse drug reactions * Reviews Datix (incident reporting software) events related to Internal Medicine services and participates in follow-up, analysis, and quality improvement efforts as appropriate * Participates in quality improvement initiatives and medication-use evaluations * Implements appropriate drug cost-containment measures while ensuring safe and effective patient care * Provides consultation to the Pharmacy and Therapeutics Committee on pharmacotherapy-related issues * Provides clinical support and guidance to pharmacy staff related to patient care and medication management * Assists in the education and training of pharmacy residents, staff pharmacists, students, interns, and technicians * Conducts research and data analysis related to Internal Medicine patient populations * Contributes to pharmacy and medical literature through therapeutic, pharmacokinetic, or pharmacoeconomic activities as appropriate Qualifications Education: To qualify a Bachelor of Science Degree in Pharmacy or Doctor of Pharmacy Degree is required. Licensing/Certifications: New York State Pharmacist License required Board of Pharmacy Specialties (BPS) certification within 1 year of hire Experience: PGY-1 Training with at least 2 years post-residency experience as a Clinical Pharmacist is required PGY-2 Training in Internal Medicine or Pharmacotherapy with at least one year experience as a Clinical Pharmacist is preferred
