**INTERNAL OPEN POSITION ONLY** JOB TITLE:  Residential Case Manager Primary Function of a Residential Case Manager:   To maintain a system in which quality service planning and documentation enhance and/or support program services, care-coordination, and public relations with our referring agencies. General Responsibilities of a Residential Case Manager: * Provide positive role-modeling of Elk Hill’s core values--respect, personal responsibility, hard-work, lifelong learning, and courage * Build and maintain positive, nurturing relationships with all youth and their families * Advocate the rights of youth * Work towards annual goals for personal, professional and educational development * Assist Program Director in training new personnel * Provide monthly administrative supervision to all residential staff as needed * Work cooperatively with staff and treatment team to improve the quality of the facility’s total program * Promote an atmosphere of openness, honesty, and understanding among residents, Elk Hill staff, and other persons associated with Elk Hill * Assist with the facility’s direct-care programming (shift coverage) as directed by the Program Director * Maintain a valid driver’s license and good standing with Elk Hill’s auto-insurance policies.         * Maintain up-to-date certifications in CPR/First Aid, and MANDT * Maintain training in DBHDS Trained Investigator Training * Demonstrate a high level of on-going competence in case management and facility programming as well as motivation and commitment as an Elk Hill employee * Within the scope of the positions job description and as delegated by the Program Director, maintain compliance with Licensing regulations and Medicaid standards * Provide routine AOC (administrator on call) support on rotating basis * Serve as “acting” Program Director when appropriate or assigned * From time to time, may be assigned to work in a group home other than the regularly assigned group home. * Participate in appropriate and ongoing supervision and staff development training * Perform other duties as designated by the facility’s Program Director   Documentation and Care-Coordination Responsibilities: * In a timely manner and in coordination with the treatment team, support the Residential Therapist in developing Medicaid and licensing compliant needs-assessment based service and discharge plans by incorporate goals and objectives that address the areas of need discovered through strengths and needs assessments * In coordination with the Residential Therapist, develop Medicaid and licensing compliant monthly progress reports for all youth capturing progress towards service plan goals and objectives, status of discharge planning, and all other areas required for monthly reporting   * Ensure the business department receives adequate and timely documentation for monthly billing purposes * As delegated by the Program Director, assist with and/or ensure various pre/post testing and assessments occur as needed * For funding purposes, develop comprehensive and timely FAPT reports that satisfy each locality’s unique documentation requirements * Arrange, facilitate, and document routine service plan meetings that encourage participation from youth, legal guardian, appropriate family members, and the youth’s placing and cooperating agents.  Professionalize these meetings with formal invites * In coordination with the treatment team, staff, legal guardian, and placing agents, develop and update the behavior support plans for each youth * Maintain the integrity of the facility’s program and medical files * Maintain an up-to-date log documenting Elk Hill’s case management services for each youth * Based on Program Director and treatment team feedback, take a lead role in the youth’s discharge process—coordinating discharge plans and aftercare services with the youth’s family, legal guardians, and placing agents.  As applicable, involve Elk Hill’s Medicaid Resource Coordinator and the Director of Elk Hill’s Community Services throughout the youth’s placement in determining the appropriateness of Elk Hill providing continued community-based services and support * Build and maintain the highest levels of professional rapport with a youth’s parents, legal guardians, and referring agencies--keeping them informed of their youth’s progress via routine phone contact, written reports, and on-site conferences * Coordinate with each youth’s family, legal guardians, referring agencies, and treatment team all youth and family visits—including home and on-site visits * Attend FAPT meetings and court appearances fully prepared with required reports, assessments, and/or other documents as needed to favorably represent Elk Hill and advocate for the needs of the facility’s youth and their families * In coordination with the Program Director and House Manager, arrange and/or provide transportation to appointments, meetings, and home visits in accordance with licensing regulations and Medicaid standards * In coordination with the Program Director and treatment team, ensure family services occur regularly Medical Files and Appointment Responsibilities (at times when there is a dedicated Medication Aid position within the facility’s organizational flow chart, the following responsibilities will be assigned to that position) * Ensure all youth receive initial and annual physical and dental appointments in timeframes required by licensing standards * Ensure all medical appointment-related documentation addresses required licensing standards and provides staff with adequate information to address the physical and/or dental needs of the youth * Ensure MARs reflect up-to-date information of both routine and/or PRN prescribed medications, treatment orders, and over-the-counter information including medications “start” and “stop” dates * Compile initial emergency packets for all youth and then update these packets as needed * Update youth’s documented medical history when new information is available * Obtain over-the-counter medication orders from a youth’s attending physician * Obtain missed medication protocols for all routinely scheduled medications * Ensure all prescriptions are filled and medications are seamlessly available to trained staff to administer them as prescribed * Coordinate all required follow-up appointments; facilitate these appointments if necessary * Work closely with the facility’s House Manager on effectively managing and documenting the facility’s adherence to proper medication administration practices * Communicate with the treatment team and staff any changes in a youth’s medication regime; physical, dental, or dietary status; or any needed updates as to the youth’s overall physical/mental health as identified by licensed practitioners Core Competencies and Experience:  * Must demonstrate sensitivity to Elk Hill’s service population’s cultural and socioeconomic characteristics * Exceptional ability to develop and coordinate a youth’s service plan and service planning needs * Strong awareness and knowledge of group dynamics and psychiatric diagnoses * Sound recordkeeping and case management abilities * Highly skilled at public relations * Essential are fluent oratory and professional writing skills * Knowledge and understanding of cultural diversity and the impact of various socioeconomic characteristics on our youth * The ability to support constructive family contact and involvement in community activities as appropriate * Ability to engage, support, and work effectively with others, including but not limited to the treatment team and other internal and external stakeholders * The temperament to work with, and care for, children and families with special needs * Sense of humor Credentials and/or Education:  * A master’s degree in social work, psychology or counseling (required), a combination of two years professional experience with children in a children’s residential facility (preferred) and in administration or supervision (preferred).  One year of clinical experience (preferred); or * A bachelor’s degree in social work, psychology or counseling, with ability to obtain QMHP, and a combination field work professional experience with children (required), in a children’s residential facility (preferred) with one year consisting of clinical experience (preferred); or * A bachelor’s degree with ability to obtain QMHP, and possess a combination of three years professional experience with children (required), in a children’s residential facility and in administration or supervision (preferred) * Must have up-to-date certification in the facility’s Behavioral Management Program and have the ability to apply such skills as needed and required in the day-to-day interactions with youth * Must have a valid certificate in an approved medication aide training class   * Good physical health and remain current with annual TB screen and other certifications * Must have up-to-date certifications in approved courses for CPR/First Aid * Maintain valid driver's license and good driving record. * Must be trained in the REVIVE!
