T INVESTIGATOR & INCIDENT COORDINATOR POSITION PURPOSE The Investigator & Incident Coordinator is responsible for coordinating, investigating, and resolving reportable incidents across assigned programs. Working under the direction of the Director of Incident Management & Investigations (PA or Regional), this role ensures incidents are documented accurately, investigated thoroughly, and resolved in compliance with regulatory requirements, RHD policies, and best practices. This position supports a culture of safety, accountability, and continuous improvement. The Investigator collaborates with program staff, leadership, and external stakeholders to ensure proper response to allegations of abuse, neglect, exploitation, and other reportable events. This role also serves as a resource to program staff by providing guidance and training on incident reporting and documentation. The Pennsylvania-based role primarily supports programs operating under Pennsylvania regulatory agencies (such as ODP, OMHSAS, and OLTL), while the regional role supports multiple states and regulatory environments. KEY RESPONSIBILITIES Incident Management & Investigation * Receive, review, and manage reportable incidents in accordance with funder and regulatory requirements * Conduct thorough investigations following state, federal, and organizational guidelines * Interview staff, individuals served, families, and witnesses using trauma-informed practices * Ensure accurate and timely completion of incident reports and required documentation * Submit reports to oversight agencies within required timelines Compliance & Documentation * Maintain compliance with regulatory reporting requirements and internal policies * Serve as a subject matter expert on incident reporting procedures and documentation standards * Monitor corrective action plans and ensure completion by responsible program staff * Maintain strict confidentiality and professionalism throughout all investigative processes Collaboration & Support * Work closely with program leadership and Human Resources regarding personnel-related concerns identified during investigations * Participate in incident review committees and contribute to follow-up planning * Serve as a liaison with external regulatory agencies when required Training & Guidance * Provide training and guidance to staff on incident reporting systems and documentation expectations * Mentor program-level incident reporters to ensure consistent and accurate reporting practices Additional Responsibilities * Travel to program sites to conduct in-person investigations as needed * Perform other duties as assigned QUALIFICATIONS Education & Experience * Bachelor’s degree required; preferred in Human Services, Criminal Justice, or a related field * Minimum 2 years of investigative experience in behavioral health, human services, or IDD settings * Experience with incident management systems strongly preferred Preferred Knowledge * Familiarity with Pennsylvania systems and agencies such as HCSIS, EIM, and ODP (for PA-based roles) Other Requirements * Valid driver’s license and ability to travel to program sites * Clean motor vehicle record for a minimum of 3 years * Successful completion of required background checks SKILLS & COMPETENCIES * Strong investigative and analytical skills * Excellent documentation and report writing ability * Ability to handle sensitive situations with professionalism and discretion * Strong communication and interpersonal skills * Ability to manage multiple cases and deadlines simultaneously Apis Services, Inc.
