An exciting opportunity has arisen for an experienced, enthusiastic and highly motivated pharmacy technician to join our Integrated Placement of Care Hub (IPOCH) in a new pharmacy role to support direct patient care and be part of our community team within Central Cheshire Integrated Care Partnership.
The IPOCH service has increased rapidly in size and now support a number of discharge services including our hospital discharge team, transfer of care hub, home care and community bed based services. This team plays a significant role in supporting the flow out of hospital and providing admission avoidance support across community.
The post holder will have a variety of knowledge and skills to support safe medicines management within the Integrated Placement of Care Hub Team (IPOCH).
This role will require enthusiasm, self-motivation, and commitment to developing a new pharmacy role within IPOCH, developing seamless pharmaceutical care for patients across traditional health and social care boundaries, and forging excellent communication between primary and secondary care teams.
The post holder will be responsible for supporting the provision of high-quality care to patients within their own homes who require medication support by the IPOCH General Nursing Assistant (GNA) service. They will be required to work across all CCICP geographical locations to support review of medication within patients’ homes. The post holder will be a registered pharmacy technician and an accuracy checker for checking dispensed prescriptions in accordance with Trust procedures.

Responsibilities
  • Medicines reconciliation for patients under the IPOCH GNA service.
  • Transcribing, and supporting the transcribing of, IPOCH GNA Medicines Administration Record charts.
  • Completing medication-related assessments to help the MDT determine if medication support is required in the community.
  • Providing patient education around medication, and assessment of medication related support aids.
  • Providing advice for patients and IPOCH GNAs on administration and handling of medicines.
  • Support the ordering and supply of medicines for patients being managed by the IPOCH GNA service, including communication with primary and secondary care.
  • Working with the MCHFT and CCICP Multidisciplinary Teams as well as pharmacy colleagues from across the wider system.
  • Supporting training and education of staff within the IPOCH service around medicines management.
  • Answering medicines-related queries within scope of competence.
  • Completing audits and participating in service improvement projects.
  • Ensuring safe practice and reducing medication associated risk.
