This is an exciting opportunity for an experienced practitioner to lead the learning disability intensive support team in Bradford. This is a well established team that actively supports individuals with a learning disability who are experiencing crisis in the community. This role involves the day to day management of the team in addition to ensuring clinical leadership and a robust timely response to crisis intervention in the community. The aim of the intensive support team is to prevent hospital admission and ensure that individuals with a learning disabilities are receiving the right support in the right place.
The successful candidate will have experience of PBS, risk management and working with individuals with a learning disability experiencing mental health crisis.

Leadership of the Intensive Support Team, within the Health Support Team

• Supervision of staff to ensure a structure is in place to facilitate safe practice and promotion of evidence base practice.
• Carry a small caseload of the highest complexity service users presenting with high levels of unmanaged risk which will include providing and receiving highly complex information and developing specialist care plans to reduce risk, support mental health and/ or reduce challenging behaviors
• Work as part of a management team in shaping and delivering a high-quality Intensive Support Team, in line with service development changes following LD Review, NDTi report and CQC feedback.
• Ensure high quality links with primary care and other services in the locality through participation in Locality integrated teams
• Working within the Mental Health Act and Mental Capacity Act in protecting the rights of vulnerable people
• Lead the adult learning disability Dynamic support register and Care and treatment review process within BDCFT, implementing the national policy and developing the local process whilst providing highly specialist advice around individuals with a learning disability who are at risk of admission to a mental health hospital. Working collaboratively with the ICB and Local Authority to ensure a multi-agency approach and local policy implementation across Bradford
