Description SUMMARY Responsible for facilitating the successful reintegration of individuals exiting the jail system into the community. This role provides support, resources, and guidance to clients, helping them navigate challenges related to housing, employment, mental health, and substance use. The Jail Transition Case Manager will work collaboratively with various stakeholders, including law enforcement, social services, and community organizations to promote a smooth transition and reduce recidivism. Position: Full-time Pay: $27.65 - $29.70/ hour Benefits: Generous Paid Time Off -- New Employees accrue up to 31 paid days off, including 15 flexible use PTO days, 10 paid holidays, 2 personal days, as well as the final week of the year.
