3. JOB QUALIFICATIONS/EMPLOYMENT REQUIREMENTS:

· Ideal candidate will have experience in working with Indigenous families and providing support and advocacy for families. They will have knowledge of both local resources, but also broader knowledge of resources available to support families in the areas of education, health and social needs.
· As this position will require some travel, including possible travel with families, a valid driver’s license, reliable vehicle and driver’s abstract is required for this position.
· Strong organizational and time-management skills.
· Strong written and verbal communication skills.
· Ability to maintain confidentiality and demonstrate professionalism.
· Attention to detail and ability to manage multiple priorities.
· Must be able to pass a criminal record check

4. WORKING CONDITIONS:

· This position will be a blend of in-office work out of Gwa’sala-‘Nakwaxda’xw Health Services, work in community and work from home. Hours will be Monday through Friday, 8:30-4:30.
· The position adheres to the personnel policies and procedures, code of ethics and current job description of the Gwasala-Nakwaxda`xw Nations.
· May require occasional flexibility for meetings or events outside regular hours.

Responsibilities

1. JOB FUNCTION/PURPOSE:

The Jordan’s Principle Service Coordination Support Worker would complement and extend the work of the Jordan’s Principle Service Coordinator by providing wrap around supports to families before and after making a Jordan’s Principle request.

2. DUTIES:

-Assist families with researching identified equipment that meets their child’s needs (i.e. laptops, medical equipment…).
· Assist families with researching and navigating the logistics of travel, such as for medical appointment. In some situations, where medical/dental travel is considered urgent and no other travel arrangements can be found, this worker may assist with providing travel.
· Provide life skills support to families, such as budget and household management, to support families to address needs around groceries, bill payments, housing, etc,
· Support families in finding and securing safe and appropriate housing.
· Support families in accessing supports/equipment after approval (i.e. grocery and clothing shopping, occasional transport support, etc).
· Assist families in determining what other supports and services could be beneficial to the family and assist in referring the families, as needed.
· Support families in having their children registered for status or other applications to benefit their children.
· Support families in applying for other financial supports such as child tax benefit, Persons With Disability, social assistance supports, etc.
· Support families in completing paperwork that may be required as part of their work with an approved service provider (i.e. autism assessment questionnaires).
-Provide assistance in navigating the logistics of complex group request applications submitted by the Jordan’s Principle Service Coordinator.
· Work in partnership with the Jordan’s Principle Service Coordinator and ARC (funding) administrator to provide wrap around supports to families and help ensure that children’s unmet needs are being addressed in a timely manner.
· Overall case management of families that are referred to this worker and other tasks as determined to support those working with the Service Coordinator to identify needs of children and youth and ensure children and youth are accessing supports and services.
