Ready2Nurse, a 5-star Medicare-rated concierge home health agency, is looking for a Junior Community Liaison to join our mission-driven team in Santa Clara County.

As a Junior Community Liaison, you’ll play a vital role in connecting referral sources, families, and community partners with our services. You’ll help promote our unique concierge model and build relationships that support seniors in remaining safely at home. You’ll work directly with healthcare professionals, senior living communities, and families to spread awareness of our services and values.
Comprehensive training will be provided to ensure you’re fully supported and confident in your role. Whether you’re new to the field or looking to grow your experience in healthcare outreach, we’re committed to setting you up for success.

QUALIFICATIONS:

  • Bachelor’s degree in Business, Healthcare Administration, Public Health, or a related field
  • Excellent interpersonal and communication skills
  • Passion for elder care and commitment to community-based service
  • Highly motivated, organized, and a team player
  • Bilingual a plus (but not required)
Responsibilities
  • Serve as a key point of contact between Ready2Nurse and referral sources, patients, and families
  • Educate the community on our personalized home health services
  • Support the coordination of care referrals and follow-up
  • Collaborate with our clinical team to ensure smooth transitions of care
  • Represent Ready2Nurse at community events and professional meetings
