Description Organization Overview: Minnesota Assistance Council for Veterans (MACV) is a statewide nonprofit organization with a mission to end Veteran homelessness in Minnesota. MACV provides comprehensive programs in the areas of housing, employment, and legal services to support Veterans and their families throughout Minnesota who are homeless or at risk of homelessness. MACV operates a budget over $14 million annually and has a team of approximately 130 employees throughout Minnesota. The organization has office locations throughout the state, with administrative functions centralized within a headquarters office based in St. Paul. Position Overview: The Justice Involved Veterans (JIV) Case Manager will utilize a network of internal and external services to assist justice-involved veteran clients in achieving compliance with community supervision, housing stability, and long-term success in the community. Veterans may experience justice involvement due to a range of complex and compounding factors, including unresolved mental health and substance use, unmet basic needs, and systemic barriers. This role is essential in providing consistent, supportive services that promote stability and reduce the risk of recidivism. The Justice Involved Case Manager shall perform these responsibilities that contribute to the MACV mission to End Veteran Homelessness in Minnesota, and to do so in ways that aspire to programmatic and administrative excellence. Primary Duties and Responsibilities: Conduct regular in-person outreach with state, federal, and county correctional facilities to identify veterans at risk of homelessness upon release. Collaborate with facility staff, MACV programs, and community partners to develop individualized reentry plans. Represent MACV and the JIV program at DOC transition fairs and other correctional events across Minnesota. This position involves travel to facilities throughout the state, which may include early start times and occasional overnight stays based on scheduling requirements. Engage with reentry teams at county detention centers to develop and implement a discharge plan for veterans exiting into homelessness. Partner with VA Justice Outreach, MDVA Justice Outreach, Veterans Treatment Court, and other veteran-serving organizations statewide to ensure coordinated support. Develop and distribute needs assessments, collect and analyze results, and identify service gaps impacting veterans reentering the community from correctional facilities. Complete client intakes, including documentation, program orientation, referral needs, and expectations. Upload documentation and enter client information and case notes into the Salesforce client management system. Maintain a caseload for post-release for the highest barrier veterans releasing from federal, state and county correctional facilities. Case management focuses on supporting veterans in maintaining compliance with community supervision following release from correctional facilities if indicated. Coordinate with housing case managers statewide to support justice-involved veterans post-release, offering case consultation and collaborative planning. Assist veterans in fulfilling conditions of release, including but not limited to: advocating for supervision transfers, registering addresses, scheduling required mental health and chemical dependency assessments, enrolling in treatment programs, and securing approval for electronic devices. Maintain accurate records of client interactions, case progress, and service coordination efforts within the Salesforce client management system. Engage with veteran service organizations, legal professionals, and community networks to enhance service delivery and advocacy efforts. Participate in professional development opportunities, including trainings, conferences, and justice-involved initiatives to remain informed of best practices. Perform other duties and administrative tasks as assigned to support program operations and the organization's mission Requirements Key Skills and Other Characteristics: Bachelor’s degree or higher with an emphasis on restorative justice, Second Chance initiatives, Social Work, knowledge of corrections, or a combination of relevant education and experience. Experience and knowledge of judicial, correctional, and community supervision conditions from a restorative justice standpoint, and veteran services within the State of Minnesota. A broad understanding of the barriers involved when an individual re-enters the community from incarceration. Must have a valid driver’s license and willingness to travel long distances. Preference to veterans and those who have experienced homelessness or justice involvement. Demonstrated ability to collaborate with others effectively, manage conflict, participate as a team member, and to give and accept criticism constructively. Comfort and/or experience working with diverse individuals of all backgrounds, including veterans charged or convicted of serious criminal offenses; to include violent crimes and sexual offenses. Ability to produce effective case notes and communicate verbally and in writing with outside agencies. Excellent organizational and time-management skills with proven ability to prioritize tasks and deadlines.
