KENOSHA HUMAN DEVELOPMENT SERVICES, INC. 3536 52nd Street Kenosha, WI 53144 Division/Department: Crisis and Stabilization Services Location: KARE Center Job Title: KARE Center Discharge Planner Reports To: KARE Center Program Manager Type of Position: Full-time FLSA Status: Non-Exempt Schedule: 40 hours per week or as demanded by the job (Monday-Friday 9am-5pm) Starting Pay: $17 per hour (may be higher based on experience) POSITION SUMMARY Implement all tasks required for the day-to-day operation of the KARE Center, to be delivered in a respectful and comprehensive manner. RESPONSIBILITIES 1. Help client implement Crisis Plan per client’s needs for all residents 2. Provide case management for clients as needed 3. Provide transportation to appointments, for clients that have no other means available to them 4. Do any necessary photocopying or filing 5. Create Discharge Plans for all KARE Center residents 6. Follow up with all residents at one week after discharge 7. Participate in the intake process by gathering required data, orienting resident to the center, opening new files on all new admissions, completing all associated paperwork and informing resident of their client rights 8. Routinely provide positive feedback and respect to residents within the center 9. Use concepts of teaching methods to avoid or diminish conflicts in the home 10. Provide supportive listening and encourage residents to follow through on their crisis plans as determined by case or crisis staff 11. Assist with follow up care, medical, mental health, and therapy appointments as needed 12. Maintain PPS and submit per month 13. Document clinical information in resident’s progress notes in a timely manner, to be completed by end of shift 14. Record discharge data and circumstances 15. Store, lock, and monitor self-administration of resident’s medication according to state guidelines 16. Provide bed checks as specified in individual files 17. Monitor each resident’s medical and emotional status and arrange emergency care when appropriate 18. Notify House Manager when an emergency or critical incident occurs, complete incident report within 24 hours and turn into House Manager, Executive Director, and Emergency Mental Health Coordinator 19. Answer house phones and respond appropriately and professionally to the needs of the caller 20. Answer Crisis Hotline and dispatch the appropriate mobile counselor 21. Participate in scheduled trainings and staffings 22. Maintain confidentiality of clients at all times 23. Be available to work some holidays 24. Complete housekeeping duties and prepare meals as necessary 25. Other duties as assigned PROFESSIONALISM 1. Conduct oneself in a professional manner a. Maintain the confidentiality and other rights of participants, families and staff and follow HIPPA Guidelines for all verbal and written communication b. Problem-solve difficult situations in a timely manner c. Manage difficult or emotional consumer situations by responding promptly to needs, requests for assistance and services d. Give, receive and act upon feedback from supervisors and co-workers in an appropriate manner e. Maintain professional boundaries with consumers and co-workers at all times f. Attend training as requested by supervisors g. Other tasks as assigned by supervisors QUALIFICATIONS: High School Diploma or equivalent required. 1. Ability to read and effectively communicate, both written and verbally, in the English language 2. Ability to traverse within the workspace and community frequently 3. Ability to frequently ascend/descend stairs 4. Ability to transport at least 30 pounds 5. Must have reliable transportation and valid driver’s license with good driving record 6. Must carry own auto insurance with at least 100,000/300,000/100,000 coverage 7. Ability to maintain records and document case notes daily 8. Ability to give and accept feedback as a part of a team 9. Be willing to learn and work with the treatment philosophies 10. Experience with AODA/Mental Health population a plus 11.
