About the Role
The ATSICHS Family Caring 4 Family (FC4F) program is a unique initiative that works to identify kin placement pathways for children and young people residing in the child safety residential OOHC system. The aim of the program is to re-unify family under safe and thriving conditions. The program is not only excluded to kin placement options but also places a great emphasis on family reunification with biological parents, when safe to do so. Coordinated and culturally safe support and interventions are provided to families. By offering a comprehensive mix of services tailored to family to community needs, we aim to build family capacity, strengthen community connections, and safeguard the wellbeing of children. At ATSICHS, through assertive kin finding, holistic case management, and collaboration with multiple service providers, FC4F empowers families to navigate challenges, develop essential like skills, and access therapeutic support in efforts of family reunification.

Duties

  • Effectively engage clients (extended family and kin of child) through cold-call home-based visits. Turn initial ‘kin agreement’ home visit into ongoing consent and interest in becoming a Kin Carer.
  • While completing family engagement activities, ask to identify other strong family members that could possibly become a kin carer. Actively seek contact details and relationship connection with child and broader family.
  • Facilitate FLDM meetings with broader family kin, gaining family consensus on a Placement Plan to reunite child with family care arrangements. Utilise the ATSIFLDM framework in facilitating meetings imbedded in the FC4F Digital Model of Care.
  • Assist in the assessment of carers, including initial screening, Blue Card submission, and PAC assessments. Advocate and support families when being assessed externally.
  • Deliver case managed services according to the program model of care, ensuring minimum case load requirements are maintained to ensure equitable access to services for community.
  • Maintain accurate and detailed case notes of all client interactions, ensuring case notes are written from the perspective of clients in their voice.
  • Ensure clients complete the ‘Have Your Sat’ client feedback tool and submit. Act on the client feedback to make continuous improvements to your practice and approach with clients.
  • Provide a service overview to various kin family members to explain the role of a Kin Carer. Be upfront in explaining challenges and complexities, alongside the positive impact and personal reward.

SKILLS AND EXPERIENCES:

Covid-19 Requirement

  • Immunisation history showing you have received two doses of a Covid 19 is encouraged.

Required

  • Bachelor’s degree in a relevant social services field with at least 4 years case management experience in family support, child protection, youth support, housing or related field.

OR

  • Diploma in relevant Community/Services field with at least 6 years in a case management role in family support, child protection, youth support, housing or related field.

OR

  • At least 6 years case management experience in family support, child protection, youth housing or related field. A commitment to complete Diploma (funded by ATSICHS) while working. And identify as Aboriginal and/or Torres Strait Islander (with confirmation proof) OR non-indigenous and having worked for an Aboriginal Community Controlled Organisation for at least 4 years.
