Description:

An exciting opportunity has arisen for a full-time Band 6 Children’s and Young Persons Keyworker within the North Central London CAMHS Children’s and Young Persons Keyworker Service based across the boroughs of Barnet, Enfield, and Haringey.
If you are interested and have a background in any of the following, e.g., Social Worker, Nurse, Occupational Therapist, Speech & Language Therapist, Psychological Therapist, Youth Worker,
Experienced Assistant Psychologists, Therapeutic/Community Workers, Special Educational Needs (SEN) staff, with a professional qualification in Health, Social Care or Education to level NVQ/HNC level 3 (or equivalent) or have the ability to complete a relevant qualification evidenced by educational history and professional development, and have experience of providing navigational support and expertise, then please apply.
1. To be a key point of contact for families/carers allocated to you, identifying gaps in their support and enabling the system to work together to bridge those gaps.
2. Where required, to provide high quality, direct support to the children and young people with complex needs and their families/carers when they need it most.
3. To work closely with Tier 4 providers, local CAMHS providers (including crisis teams), social care and education to ensure that the system is working in a coordinated and joined up way for the young person and their family/carers and helping to reduce any blockages to accessing that support.
4. Facilitate the provision and implementation of tailored behavioural and emotional support to help a child or young person and their families/carers develop skills for progress and self-management to meet their agreed goals and outcomes.
5. Communicate clearly with the child, young person and their family/carer to help them understand and navigate the system.
6. To work within and contribute to, detailed risk assessments for the young people; To ensure children and young people you support have the opportunity to make choices, develop their independence skills and access a range of positive activities both in the centre and in the community.

JO B DESCRIPTION & PERSON SPECIFICATION

Job Title: LD and Autism Keyworker
Band: NHS Band 6 WTE (£40,701- £48,054)
Department: Barnet, Enfield, and Haringey CAMHS
Location and mobility: Your normal place of work will be at St Ann’s Hospital Haringey. However, you may be required to work at any other location of the Trusts interests, including travelling to appropriate meetings, outside of the borough, as required.
Accountable to: LD/ASD Keyworker Service Manager
Responsible to: Keyworker Service Manager, CAMHS Service Managers (divisional) Trust Wide CAMHS Lead.
Responsible for : Agreed caseload.

JOB SUMMARY :

The Keyworker is part of the NHS long term plan. The vision is for all children and young people with learning disabilities and/or autism to have a designated key worker by end 2023/24. Initially the priority will be for all children and young people currently in hospital to have a key worker, then for children and young people who are at risk of going into hospital (red/amber on the Dynamic Support Registers), and with evaluation of the pilot, the aim will be to develop a Keyworking service for all children and young people with learning disabilities and/or autism by end 2023/24.
The 0-18 years Keyworker model aims to support reduction of the number of children in crisis and Tier 4 admissions, support timely assessments/formulations, provide children and families with the support that they need at the right time, improve transitions and quality of life, resulting in happy children and parents.
A key function will be to empower families and support them with navigating barriers across multiple services and organisations to effectively access the right support from the system. Our Keyworking model is about a set of functions introduced to the system via the Keyworking role, in addition to the current roles in place to support children, young people and their families. The Keyworking function will dynamically connect the system, coordinating care systemically to maximise local resources and work more effectively with families.
Each CYP will have a dedicated keyworker as their navigator, however they can access support from any co-ordinator at any one given time to link a Keyworker’s particular area of expertise with CYP needs. We recognise that our keyworkers need to be experienced to be effective in this pivotal role. We will recruit Keyworkers across social care, NHS, voluntary sectors, and experts by experience, making allowance for Local Authority and NHS working terms and conditions to be continued where applicable. Combined Health Care wish to attract a breadth of experience and relevant qualities, passion, and vision. The Keyworker will join a Professional Lead Keyworker, will work outside of the constraints of individual services (such as eligibility criteria), and have access to the CYP’s records across Social Care, Health, and Education, to achieve positive outcomes for children, young people, and their families.
