Responsibilities

· Collaborate with families and professionals to ensure our service users receive the necessary support, including liaising with GPs, clinicians, District Nurses, and LD teams
· Maintain an inventory of personal protective equipment (PPE) and conduct regular medication checks to ensure a month’s supply is consistently available.
· Complete daily logs, ABC Charts, incident and accident reports, and medication charts, ensuring adherence and accuracy.
· Assist patient’s with personal care tasks, such as washing, dressing, eating and toileting.
· Perform light housekeeping tasks, such as dusting, vacuuming, and changing bed clothes.
· Promptly inform the manager of any changes in service users’ needs, update care plans and Risk Assessments, and ensure their accuracy.
· Devise and implement a personalised weekly activity plan for each client based on their individual needs and interests.
