Guided by our values of integrity, respect, and excellence, we are seeking an experienced Lead Admissions RN to join our team. The ideal candidate for the Lead Admissions RN position handles coordination of the Admissions RN’s and Hospital Liaisons as related to the BBH Admission Process. They are responsible for educating patients/families about Big Bend Hospice and the hospice benefit, evaluating and enrolling patients as appropriate into hospice services, identifying patient/family needs and barriers to admission. Position Duties and Responsibilities: The Lead Admissions RN responsibilities will include, but are not limited to the following: Disseminates information to Admissions Team, hospital liaisons, and others requesting BBH services. Evaluates patients/families for hospice admission using approved criteria. Completes accurate, timely, and professional documentation on required forms. Communicates clearly and professionally with physicians, staff, patients, and families. Serves on-call for same-day hospice admissions. Precepts new admission nurses and hospital liaisons through their orientation. Supports ongoing education and coaching of clinical staff. Trains staff in patient assessment, care planning, and symptom management. Instructs on computer documentation and EHR use across disciplines. Demonstrates point-of-care documentation at the bedside or in-home. Provides direct patient care periodically to maintain clinical skills. Conducts field observations to coach on visits, assessments, and time management. Acts as a clinical resource for Admissions staff and hospital liaisons. Mentors and enhances clinical practice of Admissions team members. Reviews documentation to ensure billing readiness. Manages non-admissions and ensures timely follow-up. Mentors new team members to ensure consistent onboarding and performance. Schedules visits, addresses barriers, and retrieves medical records as needed. Coordinates patient transport and travel into BBH service area. Shares information on community resources when BBH care isn't suitable. Provides feedback to referral sources, physicians, and families. Maintains regular contact with referral sources to support intake process. Collaborates across departments to ensure an efficient intake program. Maintains confidentiality of sensitive information. Uses resources responsibly. Performs additional duties as assigned Work Schedule: Monday-Friday: 9:00 am- 6:00 pm. Rotating Holiday’s and weekends Skills & Qualifications Education & Experience: Associate degree in Nursing. Must have 1 year of nursing experience in hospice health care. Three years of nursing experience including 2 years of “hands on” clinical experience such as providing care, supervising nursing services or teaching nursing skills in an organized curriculum or in-service program. Skills & Abilities: Excellent verbal and written communication skills. Excellent interpersonal, negotiation, and conflict resolution skills. Excellent organizational skills and attention to detail. Strong analytical and problem-solving skills. Ability to prioritize tasks and to delegate them when appropriate .Ability to act with integrity, professionalism, and confidentiality. Thorough knowledge of employment-related laws and regulations. Proficient with Microsoft Office Suite or related software. Proficiency with or the ability to quickly learn the organization’s HRIS and talent management systems. Most services are provided within regular working hours, but some evening and weekend hours may be required. Requirements: Valid driver’s license, auto insurance, and reliable transportation.
