Description

Hours: 40 hours per week. Monday - Friday, 8:00 am 4:00 pm (no on-call, evenings, or weekends).
Pay Rate: $26.93 to $28.35 per hour
Education: Bachelor’s degree in Public Health, Social Work, Psychology, Human Services or related field with 2 years of experience working in social services or related setting required. Master’s degree or enrollment in Master’s program in related field with 1 year of experience working in social services or related setting strongly preferred.

As a Lead Care Manager II, you will:

  • Provide mobile outreach and engagement, including program capacity building
  • Screen and assess for program eligibility
  • Promote positive health outcomes for clients utilizing mobile case management, care coordination with other providers, and supportive services with a team of dedicated professionals
  • Connect clients and their families to a medical clinic and providers, as well as other clinic and community-based services
  • Serve as an advocate for the client and family
  • Use motivational interviewing, trauma-informed care, and harm-reduction practices
  • Create and maintain all required documents in the electronic and paper record systems

ADDITIONAL QUALIFICATIONS AND SKILLS

  • Experience working with individuals with serious mental illness (SMI) or other chronic health conditions preferred
  • Proficient computer skills, including MS Office
  • A valid California driver’s license in good standing, personal vehicle, proof of insurance and eligibility to be an approved CRF driver is required to fulfill the essential functions of this position.
  • Ability to pass criminal/background clearance.
