Lead Care Manager (ECM) – Hybrid Role Heritage Health Network (HHN) Join Heritage Health Network and make a meaningful impact in your community as a Lead Care Manager supporting members through our Enhanced Care Management (ECM) program. This is a hybrid position (2 days remote, 3 days in the field), giving you the flexibility to work from home while building strong, in-person connections with members across Low Desert regions.
