This position will be located at 233 Main St. New Britain, CT 06051. Monday-Friday, 9:00am-5:00pm.    Compensation Range: The salary range for this position starts at $21.00 per hour. Compensation for each position is determined not only by years of relevant experience, education, and skills, but also by maintaining internal pay equity. CMHA also takes into consideration our benefits and paid time off package to provide a well-rounded and competitive approach to the applicant’s overall compensation.   Position Summary: The Case Manager assists in overseeing program functions/operations in accordance with policies, procedures, internal processes, and external regulatory agencies, including but not limited to DPH, Joint Commission, DMHAS, DCF and Grant/Payer Sources under the supervision of Program Coordinator. Provides coaching to staff, as well as ensuring appropriate delivery of services to adult clients. Responsible for ensuring treatment is person-centered and recovery oriented. Responsible for a caseload of clients. Serves as an intensive case manager to support, teach, advocate for individuals with mental health, primary care and/or co-occurring disorders.   Essential Responsibilities: * Maintains appropriate caseload as determined by CMHA and funder • Provides direct care to clients by conducting individualized and group education, skill building, recreation and household maintenance activities and/or monitoring environment with clients for cleanliness, safety and repairs. * Works with clients to teach life skills necessary to live independently in the community. This includes assisting with finances and entitlements, supporting the development of activities of daily living, nutrition, housing maintenance and repair, and prompting to encourage task completion. * Educates clients to utilize emergency and community resources to help meet basic needs. * Develops and reviews individualized treatment plans with clients to identify goals and progress throughout the client’s stay by meeting with each client on caseload in adherence to program requirements * Assures timely documentation is maintained in accordance with CMHA policies and procedures, including but not limited to progress notes, treatment plans, authorizations, program orientation, consent to treat and monthly targeted case management notes. Completes all additional program required documentation, including shift report, in compliance with CMHA policies and procedures. * If applicable, supervises and documents the self‐administration of medication to program clients as scheduled in an accurate, professional manner by adhering to program policy, procedure and training. * Collaborates and integrates with other CMHA teams to ensure all services provided to client are communicated with client’s treatment team. * Communicates with external providers to ensure consistency of and accessibility to requested/needed services. May provide transportation as needed. • Provides coaching and administrative support to staff to ensure appropriate delivery of services to all clients. * Participates in and documents staff meeting minutes to ensure timely and accurate communication related to client care, facility cleanliness, safety, and other matters. * Oversees day-to-day scheduling and provides individual and/or group activities to ensure program coverage as indicated by program schedule and compliance with regulatory and payer sources. * Participates, as a CMHA representative, in all necessary internal and external meetings that pertain to clinical care, compliance and client activity. * Performs other related duties as assigned Requirements: * Bachelor’s Degree in a mental health related field preferred or equivalent experience. * Successful experience working in Case Management or related field with a solid understanding of Mental Health conditions, preferably a minimum of one year. * Successful experience in the provision of integrated care of clients with severe/prolonged mental illness and/or co-occurring disorders or equivalent, preferably a minimum of 1-3 years. * Successful experience in a leadership or supervisory role of direct care providers to clients with severe/prolonged mental illness or equivalent, preferably a minimum of 1-3 years. * Must have valid Connecticut driver’s license, a reliable vehicle. * Will become certified in MANDT, First Aid, and CPR. * Participate in trainings on an annual basis, as assigned.   We care about our employees by offering benefits that strike a harmonious work-life balance.
