Here is what you will be doing:
The Provincial Palliative Care Program (PPCP) at Ontario Health collaborates with delivery partners to achieve the Ministry of Health’s palliative care priorities, as outlined in the Ontario Provincial Framework for Palliative Care. This role is crucial in implementing the framework, developed under the Compassionate Care Act, 2020, ensuring equitable access to high-quality palliative care across all settings.
The Lead will advance the provincial palliative care education strategy and health service delivery framework by developing and implementing the Provincial Education Plan, including development of clinical guidance and educational materials. This position integrates Models of Care into the provincial delivery framework, supports digital and virtual initiatives, and ensures that education, clinical guidance, and system-level strategies are evidence-informed, person-centered, and aligned with provincial priorities.
This foundational work guides the responsibilities outlined below, and drives continuous improvement in palliative care delivery.
