Join our team today and make a difference!   Pay: Starting at $28.19/hour ranging to $29.37/hour for qualified experience SCHEDULE: Monday through Friday, 2:00pm- 10:00pm Total: Full-Time (40 hours/week) SCOPE OF RESPONSIBILITY The Lead LPN is responsible for overseeing the health and wellbeing of the residents and medication administration under the oversight of the RN Supervisor/Manager as well as the supervision of the shift under the Residential Manager.    WORKING RELATIONSHIP This position reports directly to the Supervising RN/Manager. Develops and maintains relationships with the following: 1. Program Managers 2. Direct Support Staff 3. Office Administrative Staff 4. Agency Nursing Staff 5. Program Director 6. Health Care Consultants 7. Regulatory Agencies 8. Community (neighboring businesses, etc.) 9. Maintenance Staff   QUALIFICATIONS 1. Valid Nursing Degree/License 2. Good interpersonal relationships, verbal communication skills 3. Good judgment and ability to handle confidential information 4. Ability to handle stressful and/or crisis situations 5. Good time-management skills (ability to meet deadlines and prioritize multiple tasks) 6. Must hold a valid CT driver’s license and be insurable with Network’s insurance carrier   JOB RESPONSIBILITIES 1. Ensure nursing procedures are in compliance with all agency and DDS policies 2. Carry out the philosophy, goals, and objectives of Network, Inc., while following all governing regulations as they pertain to group homes and intellectually and physically disabled individuals 3. Responsible for all medical care for residents in Network’s group homes 4. Receive and implement MD Orders and ensure that they are followed. Report errors as needed 5. Order resident medications per MD orders 6. Administer medications 7. Monitor residents’ vitals and weights 8. Review nursing care plans 9. Assist hospital admission and discharge planning of new and current residents in order to ensure that all medical needs are met 10. Track medical appointments to ensure all medical needs are being met 11. Attend resident medical appointments 12. Collaborate with outside medical professionals 13. Ensure that special diets/consistencies are being followed and prepared properly. 14. Assist residents with all ADL tasks as needed 15. Prepare kardexes and perform daily review of individuals’ kardexes and MD orders for compliance 16. Provide medical monitoring and manage symptoms as needed for residents 17. Complete documentation and follow-up as required 18. File medical documents and maintain orderly medical records for all individuals 19. Maintain general cleanliness of home and restocking of supplies  20. Assist RN supervisor when needed 21. Maintain good communication between residence, day programs, coworkers and supervisors on medical needs of the residents 22. Ensure program compliance with medical protocol through timely and accurate reporting of medical data, both internally and externally as needed 23. Supervise and train direct care staff as designated by Manager 24. To be a role model for direct care staff 25. Consult with Manager weekly on operation of the house 26. Replace and refill all forms; review financial, program and medical data  27. With Manager, arrange and monitor schedules and staffing patterns including emergency coverage/shift coverage. 28. Oversee resident programming, recreation  29. Perform other duties as required by the RN Supervisor/Manager   EDUCATION AND EXPERIENCE Graduate of an accredited Licensed Practical Nursing Program and maintains a current LPN license through the State of CT.
Responsibilities
