Position Summary The Lead Registered Nurse provides direct patient care while serving as a leader within the clinic. This role supports practitioners with patient care, physical examinations, and ancillary tests, while utilizing the nursing process and evidence-based practice to coordinate and enhance patient outcomes. In addition to clinical responsibilities, the Lead RN provides mentorship, workflow coordination, and operational support to ensure high-quality, patient-centered care and efficient clinic operations. Core Responsibilities Deliver comprehensive patient care by supporting practitioners in the management of acute and chronic conditions, including participation in physical examinations, coordination of care, and execution of ancillary testing. Perform specialized nursing assessments, including triage, blood pressure checks, injections, and insulin education (telephonic and in-person) Triage unscheduled patients requesting same-day care by identifying acute needs and mobilizing appropriate care team resources Coordinate care transitions both internally and externally to meet patient needs Monitor VA discharge lists and conduct timely patient follow-up after hospital or facility discharges Provide patient and family education focused on self-management, prevention, and wellness aligned with patient health goals Support management of patients requiring additional care, including those with chronic conditions Maintain accurate and timely documentation in the EMR Maintain confidentiality of veterans’ data and information Patient Aligned Care Team (PACT)Coordination & Collaboration Serve as a key point of contact within the PACT Team Coordinate daily team huddles, identifying high-acuity and at-risk patients Facilitate communication and care coordination across interdisciplinary teams Leadership & Staff Development Serve as a clinical leader, mentor, and resource to nursing staff Lead regular nursing staff meetings to promote communication, education, and engagement Provide guidance on clinical workflows and best practices Assist in onboarding and training of new nursing staff Support the administrator in maintaining staff competencies and professional development Act as backup in the absence of the Clinic Administrator Quality, Compliance & Clinical Outcomes Ensure compliance with Office of Inspector General (OIG), Joint Commission (JC), Environment of Care (EOC), lab, and other regulatory requirements Adhere to all VA and company training requirements Support clinical quality initiatives and continuous improvement efforts Maintain focus on achieving excellent clinical outcomes in alignment with VA guidelines Consult with VAMC as permitted Operations & Customer Experience Provide exceptional customer service to veterans, colleagues, and visitors in person and over the phone Foster a warm, welcoming, and professional clinic environment Collaborate with clinic leadership to resolve patient concerns or complaints Support clinic operations and workflow efficiency Strategic & Program Support Participate in clinic outreach events to support enrollment growth initiatives Support patient enrollment, retention, and follow-up care (Save & Recapture efforts) Embrace and support new clinical and operational initiatives Requirements Qualifications Bachelor of Science in Nursing (BSN) required; an Associate’s Degree RN may be accepted if permitted by VA contract Minimum 2–3 years of RN experience in primary care, ambulatory care, or related setting; government healthcare experience preferred Active, unrestricted RN license in good standing (state requirements per VA contract) Must be credentialed and remain in good standing with the Veterans Health Administration (VA) Current Basic Life Support (BLS) certification; ACLS as required by VAMC contract Proficiency in written and spoken English Strong computer skills, including EMR experience Additional Attributes Strong leadership, communication, and organizational skills Ability to mentor, guide, and support clinical staff Energetic and optimistic demeanor Strong service mentality with commitment to patient-centered care Ability to balance clinical responsibilities with leadership duties Work Environment & Expectations This is a safety-sensitive position The Lead RN is expected to function as a working leader, providing both direct patient care and team leadership May be required to step into staffing or leadership roles as needed to support clinic operations Final compensation will be based on relevant factors including work experience, skills, certifications, and internal equity.
