Summary of Duties and Responsibilities: The Annual Wellness Visit Nurse (AWV) Lead works in collaboration with the primary care providers and all members of the health care team. Primary responsibilities includes supervision of other AWV RNs, active participation as part of the Population Health Leadership Team, tracks metrics related to AWVs, collaborates across disciplines to promote education and uptake of AWVs. Also functions as an AWV Nurse and has responsibility for identification of Medicare patients appropriate for annual wellness visits and execution of guided processes to complete the annual wellness visits. The Annual Wellness Visit Nurse must be knowledgeable of community resources and assists patients in connecting with those resources, serves as patient advocate, and assists in identification and improvement of service delivery. This position requires expertise in the nursing process and using critical thinking skills to plan and coordinate care. Responsible and accountable for direct and indirect patient care for designated patient populations. Primary Duties and Responsibilities: Supervise and train AWV RNs. Designs and provides training with the Population Health Leadership Team to other Variety Care staff in their roles in the AWV process. Tracks metrics for AWVs. Assures processes are in place for all AWV types and components. Attends required Population Health Leadership team meetings. Reviews clinic schedules and reports to identify patients appropriate and due for Medicare annual wellness visits. Works collaboratively with clinical teams to accomplish annual wellness visits in an efficient manner for patient and team. Builds positive, open communication between patients, caregivers, providers and staff, and works as a team member to enhance the patient experience. Follows annual wellness visit protocols and standing orders including completion of preventive services that may result from the annual wellness visit. Acts as patient advocate. Develops and shares knowledge of annual wellness visit eligibility and documentation requirements. Develops knowledge of Medicare preventive services and clinical guideline recommendations for those services. Addresses open quality gaps with patients and works collaboratively with patient towards closure of those gaps. Documents within the electronic medical record to satisfy closed quality gaps. Reviews and adheres to all Medicare guidelines regarding the performance of Annual Wellness Visits. Performs periodic chart audits to ensure visit quality Follows universal precautions and all safety regulations; uses proper PPE as task on hand requires. Follows HIPAA guidelines and Variety Care policies and procedures as they relate to privacy and confidentiality of patients and staff. Serves as a professional role model for all staff. Assists with medical emergencies when in clinic, including Code Yellow and Code Blue. Conducts staff training related to emergency situations. Assists with emergency protocol development and drills. Qualifications Requirements, Special Skills or Knowledge: Current RN license in the state of Oklahoma. Bachelor’s degree in nursing. ADN degree considered. 5 plus years of previous experience in an outpatient clinic setting. Current BLS-HCP certification. Leadership or supervisory experience. Strong leadership and decision-making skills. Strong oral and written communication and computer skills. Preferred Requirements, Special Skills or Knowledge: Healthcare management experience. Annual Wellness visit experience. Bilingual (English/Spanish). Essential Functions: Able to lift at least 25 pounds. Able to assist patients getting onto and off of exam tables.
