Description

We are recruiting for a full time band 5 learning disabilities nurse to work in our health facilitation team. You would be based in the community, as part of our multi disciplinary team with the overall aim of reducing health inequalities in adults with learning disabilities. This may include supporting people with their physical health, their emotional wellbeing and / or their mental health.

The post holder will work collaboratively with adults with learning disabilities, their carers, professionals and agencies to provide a high quality, comprehensive and dynamic specialist nursing service with the central aim of maintaining and improving the health and wellbeing of adults with learning disabilities. This will include but is not limited to, working directly with service users who have complex healthcare needs, supporting others to act as Health Facilitators, supporting health providers to make reasonable adjustments to care pathways and engaging in a range of health promotion activities.
You will need to be able to manage a caseload, balance competing demands, ensure information is shared with people in appropriate accessible formats, and work independently.
Ideally we want applications from learning disabilities nurses - however if you are a different branch of nursing and have knowledge and experience of supporting people with learning disabilities alongside a passion to reduce health inequalities for people we would consider your application.
