Job Title: Leaving Care and Education Co-ordinator
Location: Head Office, Langley
Salary: Up to £30,000 per annum + benefits
Hours: 40 hours per week
An exciting opportunity has arisen for a Leaving Care and Education Co-ordinator to join the team at Sunbeam Fostering. This role will be based at our Head Office in Langley. You’ll share our commitment to provide a high-quality service to achieve positive outcomes for our children/young people.
Sunbeam Fostering may provide a work permit for the right candidate if necessary

REQUIREMENTS

  • Minimum 1 year experience in Similar role (desirable)

Responsibilities

WHAT THE ROLE INCLUDES:

· Identify young people turning 18years – following pathway planning processes, care plans and applying policy and procedures for Sunbeam and LA’s
· Circulate information to Foster Carers including guidance on staying put arrangements, process and expectations
· Communicate with relevant professionals in regard to the staying put plans (LASW, Commissioning, SSW and Placements Team) including attending meetings to support leaving care planning.
· Keeping up to date records in relation to staying put planning, including case records, liaising with accounts and confirming payment arrangements for foster carers.
· Supporting with training and development, through 1:1 support discussion with Foster Carers, supporting and facilitating workshops for foster carers and staff team
