Social Worker : Adult Assessment & Care Management Team - Permanent Band 6
An exciting opportunity has arisen to work as a Social Worker 18.5 hours per week in a Community Adult Social Care Team.
You will be working within a busy assessment and care management team. You will work closely with our partner multidisciplinary teams. The post will be based at Edwin House, Burton Upon Trent, you will be joining a multi skilled Team consisting of Social Workers, Social Care Assessors, Occupational Therapists & Sensory Specialists.
You will undertake assessment & care management responsibilities under the Care Act 2014 and have a good understanding of MCA Act 2005. The Trust is committed to safeguarding and promoting the welfare of children and young people/ vulnerable adults and expects all staff to share this commitment. Part of your role will be to contribute to work with Vulnerable Adults.
The service Users, families and carers we work with may have safeguarding concerns identified, your role may include being part of leading planning discussions & completing section 42 enquiries, safe & well visits , signposting to other relevant partner agencies as required.
You will be working in partnership with colleagues /other professionals and have a good knowledge of the services and equipment now available to support independence in the home environment would be desirable .
You will be responsible to the Team Lead and will have support from other colleagues within the wider Team.
You will need to have a full driving licence and access to a car. NHS car scheme available.
