Description

At NorthBay Health the LCSW provides direct clinical social work service, counseling, discharge planning, and case management services to hospital patients and their families including biopsychosocial assessment and documentation of emotional, psychological, social, financial, environmental or follow-up referral needs. Ability to initiate and discontinue 5150 custodial holds WITHIN NorthBay Medical Center and VacaValley Emergency Rooms, and inpatient units. (5150 Custodial Hold: Welfare and Institutions Code section 5150 et seq. specifies the circumstances under which a person who is gravely disabled or a danger to themselves or others as a result of a mental illness may be detained against their will for psychiatric evaluation and treatment for up to 72 hours). Works collaboratively with interdisciplinary healthcare staff. Manages the psychosocially complex patient across the continuum of care by completing a clinical bio-psychosocial assessment, formulating a plan of care related to assessment of patient’s unique strengths and needs, developing a discharge plan, monitoring patient’s progress toward the goals of care, and identifying barriers to meeting estimated length of stay. Facilitates integration with community-based resources to optimize patient health. Collaboration with community partners to optimize patient health. Communicates appropriate information to care providers in the continuum of care. Documents assessment and interventions in patient’s medical record.
