This role delivers outpatient therapy, conducts assessments, coordinates care with medical providers, and supports patients in achieving better health outcomes through evidence-based practices. The therapist ensures services meet FQHC, state, and federal regulatory requirements, including documentation, productivity standards, and culturally responsive care. Key Responsibilities Clinical Services Provide individual, family, and/or group psychotherapy using evidence-based interventions appropriate for diverse populations. Conduct comprehensive behavioral health assessments, including psychosocial evaluations, risk assessments, and diagnostic impressions (per DSM-5-TR). Develop and implement person-centered treatment plans with measurable goals. Offer crisis intervention, safety planning, and referrals for higher levels of care when needed. Provide brief, focused interventions in collaboration with primary care providers as part of integrated behavioral health. Care Coordination Collaborate closely with medical, dental, case management, and care team members to support whole-person care. Participate in interdisciplinary team meetings, huddles, and case consultations. Refer patients to internal and community resources such as psychiatry, substance use programs, social services, and housing support. Support patients in navigating healthcare systems, reducing barriers to engagement and adherence. Documentation & Compliance Complete timely and accurate progress notes, assessments, treatment plans, and other required documentation in the Electronic Health Record (EHR) per FQHC standards. Maintain compliance with federal, state, and HRSA regulations, including UDS reporting needs. Uphold confidentiality and ethical standards consistent with HIPAA and professional licensing boards. Patient Engagement & Community Support Provide culturally competent and trauma-informed care to patients from underserved and diverse populations. Participate in outreach, quality improvement initiatives, and population-health-focused behavioral health strategies. Support integration of behavioral health screenings (PHQ-9, GAD-7, ACEs, SUD tools, etc.) into routine patient care. Requirements Qualifications Required Master’s degree in Social Work, Counseling, Marriage & Family Therapy, Psychology, or related field. Current, active licensure in the state of practice (e.g., LCSW, LPC, LMFT, LMHC, LPCC). Minimum of 2 years clinical experience providing therapy in outpatient or community-based settings. Experience working with diverse and underserved populations. Knowledge of evidence-based therapeutic modalities such as CBT, DBT, MI, trauma-informed care, and solution-focused therapy. Strong documentation, communication, and time-management skills. Preferred Experience working in an FQHC, community health center, or integrated care setting. Bilingual proficiency (e.g., Spanish) highly desirable. Familiarity with Medicaid/Medicare, value-based care, SUD treatment, and collaborative care models. Work Environment Full-time or part-time position within a multidisciplinary FQHC setting. May involve hybrid onsite/telehealth delivery depending on organizational policies. Must be able to manage a diverse caseload and meet productivity expectations while maintaining high-quality care. Physical & Other Requirements Ability to sit for extended periods and use EHR systems. Successful completion of background check, credentialing, and tuberculosis screening.
