Summary
This role is ideal for a licensed counselor who is passionate about client-centered behavioral health care. You will lead coordinated care efforts across mental health, substance use, and social service domains. Key responsibilities include designing and implementing treatment plans, conducting assessments, providing therapy, and collaborating with a multidisciplinary healthcare team to ensure client safety and success.

What You’ll Do

  • Develop and implement individualized treatment plans based on client needs and team input.
  • Provide individual, family, and group counseling using evidence-based practices.
  • Coordinate care by linking clients to medical, housing, financial, and legal services.
  • Maintain accurate, detailed documentation in compliance with legal and agency standards.
  • Lead engagement strategies, safety planning, and recovery-focused interventions.
  • Collaborate with physicians, nurses, case managers, and external service providers.

What We’re Looking For

  • Master’s degree in Counseling, Social Work, Psychology, or a related field (required).
  • Active clinical license or eligibility preferred (e.g., LADC, LPC, LCSW, LMFT, LPCA, LMSW, CADC).
  • Experience working with substance use, mental health, and/or co-occurring disorders.
  • Knowledge of FQHC or integrated care models is a plus.
  • Strong communication, documentation, and clinical assessment skills.
