HTS is seeking a Licensed Occupational Therapy Assistant to lead our therapy team at a highly rated continuing care retirement community in Fort Wayne, Indiana. This role offers the opportunity to provide skilled treatments for a majority of the work hours while also performing Lead Therapist duties. Treat a diverse caseload from short term rehab, long term care, assisted/independent living and memory care. Key Responsibilities: · Provide high-quality occupational therapy to residents in a supportive environment. · Collaborate with a multidisciplinary team to develop and implement individualized treatment plans. · Lead Therapist duties include attending facility and team meetings, following up on therapy referrals, planning minutes and reporting any issues to Regional Director. Requirements Applicants must have an active Indiana Occupational Therapy Assistant license or must have the necessary qualifications to earn Indiana license. These qualifications include a degree from an ACOTE-accredited occupational therapy assisting program, subsequent passing of the National Board of Certification in Occupational Therapy (NBCOT®) examination, and fulfilling the state’s requirements for licensure.
