Description Licensed Practical Nurse (LPN) - Skilled Nursing We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our skilled nursing team. Schedule is 7p-7a working 2 nights a week and every other weekend Key Responsibilities: - Provide direct patient care under the supervision of registered nurses and physicians - Monitor and document patients' health status, including vital signs and symptoms - Administer medications and treatments as prescribed - Assist patients with activities of daily living, such as bathing, dressing, and mobility - Collaborate with healthcare team members to develop and implement care plans - Educate patients and their families on health management and post-discharge care - Maintain accurate and detailed patient records in compliance with healthcare regulations - Ensure a safe and clean environment for patients and staff Skills and Qualifications: - Valid and current Licensed Practical Nurse (LPN) license in the state of practice - Proven experience in a skilled nursing or long-term care setting preferred - Strong clinical skills and knowledge of nursing practices - Excellent communication and interpersonal skills - Ability to work effectively as part of a multidisciplinary team - Compassionate, patient-focused approach to care - Ability to handle multiple priorities and remain calm under pressure - Basic computer skills and familiarity with electronic health records (EHR) systems Join our team and become part of a caring organization that values professional growth, teamwork, and excellence in patient care. Requirements Qualifications 1 Must be a Licensed Practical Nurse in good standing and be currently licensed by the Commonwealth of Pennsylvania Bureau of Professional and Occupational Affairs, Department of State; or 2 Must be knowledgeable of nursing and medical practices and procedures and laws, regulations, and guidelines governing skilled nursing facilities; and 3 Must be at least eighteen (18) years of age and completed high school or obtained the equivalent of a high school education; and 4 Must possess the ability to speak to and be understood by residents and staff, and demonstrate the ability to read, write, and follow oral and written directions; and 5 Must possess an agreeable and helpful attitude toward senior adults.
Responsibilities
The Licensed Practical Nurse will provide direct patient care under supervision, monitoring and documenting health status, administering prescribed medications and treatments, and assisting patients with daily living activities.
