Your Opportunity:
Reporting to the Care Manager, the HCA Coordinator will be responsible for the ongoing education and training of the HCA’s working in the Rural West Canmore and Banff Integrated Home Care Calgary Zone offices, while remaining competent in full scope of practice. The HCA Coordinator will work collaboratively with the HCA Supervisor, Care Manager and the team within the Rural West IHC program. The HCA Coordinator key roles will include: Monitor all aspects of the care assigned to the HCA – including home visits. Including but not limiting to the MAP program, personal care, restricted activities and clinical documentation. Coordinating orientation for new HCA hires. Identify Ongoing educational needs for HCA including Competency Assessment profiles. Coordination of Educational days for the HCA. Coaching and mentoring HCA in their competencies to perform duties. In addition, The Licensed Practical Nurse (LPN) in Home Care works as a member of an inter professional health care team utilizing the nursing process to provide case management and direct nursing care to individuals across the life span. The LPN is responsible for their nursing practice as defined by the College of Licensed Practical Nurses of Alberta (CLPNA). In collaboration with the Registered Nurse Home Care Case Manager, the LPN is responsible to assess, plan, implement and evaluate the needs of Home Care clients and their caregivers/families. The role provides services to clients in home, lodge, and clinic settings. Please note this position requires travel to and from Canmore and Banff home care offices and to clients within these geographical areas.
