General Purpose: The Licensed Practical Nurse will provide quality nursing care in the department, as directed by the departmental lead nurse. Description of Duties: Perform screening procedures and documentation in accordance with Cornerstone policies and procedures. Identify potential patient problems, alert the RN or Provider and anticipate orders for intervention. Perform screening procedures according to Cornerstone policies. Assist providers with in-office procedures and exams as needed. Participate in Cornerstone’s Care Team Model including but not limited to: § Participate in Pre-Visit Planning (see Pre-Visit Planning Policy). § Lead care team huddles/communication. § Direct population management activities for assigned patient panel. § Direct and participate in quality improvement activities within the care team and department. Administer medication, treatments and vaccines according to provider orders and scope of practice. Perform hearing and vision screenings on children and adults. Responsible for carrying out Medical Providers orders for the following actions: EKG’s, Emergency response, medication administration (Oral Medications, vaccinations, prescribed treatments) and other procedures according to Provider orders. Strictly adhere to safe nursing practice, including but not limited to, Infection Control, National Patient Safety Goals, Medication Management and Waived Testing set forth by Cornerstone policy and incompliance with Joint Commission and OSHA. Refer phone calls and patient triage to Lead Nurse or Physician. Float to other departments and the laboratory as needed. Make recommendations to supervisors for the improvement of health care to all patients. Ensure a clean and safe patient environment. Be familiar with, and adhere to, all clinical, administrative, and general policies and procedures as outlined in the Cornerstone Policy and Procedure Manual and Employee Handbook. Attend and participate in monthly department, All Staff and other required meetings. Maintain confidentiality of all aspects of Cornerstone including, but not limited to, patient confidentiality, financials, and employee relations. Perform other duties, as assigned. Requirements Diploma from an Accredited LPN program/school Current BLS Certification Current NYS Licensed Practical Nurse SCHEDULE: Monday: 11AM-7PM Tuesday-Friday: 9AM-5PM One Saturday a month: 9AM-1PM
