Overview CVP seeks Licensed Practical Nurses to join our team in providing our Nation's Veterans with nursing services at the VA Palo Alto Health Care System, Menlo Park campus, located at 795 Willow Street, Menlo Park, CA 94025. This position supports the Community Living Center with potential assignment to the main Palo Alto campus as operational needs dictate. Pay: $35-40/hr Responsibilities Provide basic nursing care to patients/residents under the direction and supervision of Registered Nurses or physicians, including assistance with activities of daily living (bathing, dressing, feeding, toileting, and mobility assistance). Monitor and record patient vital signs (temperature, blood pressure, pulse, respiration), administer prescribed medications, document medication administration accurately, and perform basic wound care including dressing changes. Assist with specimen collection and delivery to laboratory, monitor patient/resident status, report changes in condition to supervising RN or physician immediately, and document care activities accurately and timely in the electronic medical record. Assist with admissions, transfers, and discharges; help patients/residents with mobility including transfers, ambulation, and use of assistive devices; and maintain a clean, safe, and organized patient care environment. Follow infection control procedures, maintain standard precautions and PPE compliance, respond appropriately in emergency situations, provide basic life support as needed, and participate in fall prevention and pressure injury prevention programs. Practice VA core values (ICARE), report abuse incidents immediately to Nurse Manager or designated supervisor, clear assigned alerts in electronic health record, and complete all mandatory VA training requirements. Qualifications U.S. Citizenship required; ability to successfully undergo Government sponsored background investigation. Successful completion of an accredited practical nursing program of at least 9 months' duration evidenced by certificate or diploma, OR completion of a 2-year degree program in practical nursing from an accredited institution. Current, full, active, and unrestricted Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license in any U.S. State, Territory, Commonwealth, or District of Columbia; must maintain throughout employment. Minimum 2 years recent LPN experience. Current nursing license with no negative indicators. Experience in geriatric or long-term care settings preferred. Current Basic Life Support (BLS) certification from American Heart Association or American Red Cross required; must maintain throughout employment. Completion of standard infection control measures including current PPD, influenza vaccination (unless contraindicated), and blood-borne pathogen training; ability to speak, understand, read, and write English fluently. Physical ability to sit, stand, walk, lift up to 50 lbs from floor to waist, carry up to 40 lbs, and push/pull with 30 lbs of force during work shift. Shift Shifts include 7:00 AM - 3:30 PM; 7:30 AM - 4:00 PM; 8:00 AM - 4:30 PM; 10:30 AM - 7:00 PM; 11:30 AM - 8:00 PM; 12:30 PM - 9:00 PM; 3:30 PM - 12:00 AM.
