Job Summary: The LPN provides 1:1 skilled care to clients in the community, at their home and school setting. Location is for in person at the Carbondale School District. This position starts and ends at the patient’s school.
We support you by providing a home office for questions and on-going training.
RN care coordinator to answer your clinical questions and concerns
Quality Assurance program to assist you with documentation and up to date compliance information

QUALIFICATION & REQUIREMENTS:
Good Standing Licensed Practical Nurse
6 Months of experience in Adult or Pediatric Care
TB skin test (must be within 12 months)
· CPR certified
· Pediatric experience is helpful but not mandatory, We train all nurses
· Completes all required regulatory requirements.
· Implements infection control and safety practices according to agency policies.
· Adheres to OSHA guidelines.

Job Type: Full-time and Part -time available
· Provide 1:1 Skilled care for clients in the Community, Home and School setting
· Health promotion, teaching, and training of client and family members.
· Assess and observe patient’s condition and provide ordered care
· Coordinate with RN Care Manager to promote accurate and updated Plan of care
Completes all required documentation per Agency and State Standards
· Completes all required regulatory requirements.
