Job Summary: The Clinical LPN I will provide clinical support to the patients assigned to the Pod as needed. The Clinical LPN I will have worked in their position for at least one year and may follow the clinical ladder guidelines for advancement to Clinical LPN II. May be assigned to special projects such as primary care/mental health integration or to a satellite site. Demonstrates a sincere dedication and loyalty to the mission, vision and core values of BRMC. Duties include performing as a member of the team and assisting in the administration and maintenance of an efficiently operated clinic. Primary responsibility will be care management, including:

Responsibilities:

  • Manages a population of assigned patients and focuses on the execution of treatment plans and protocols.
  • Assists with the development of care management methods and practices.
  • Effectively uses care management information systems.
  • Reviews patient charts to analyze data and develop a plan of care consistent with patient health status.
  • Assesses the level of care that will be needed for the patient visit prior to the patient appointment.
  • Documents all patient care provided in the patients? electronic health record.
  • Communicates tests results with patients.
  • Assists with nursing visits including blood pressure readings, medicine reviews, maintenance injections etc.
  • Assists referral coordinators with arranging patient referrals to specialty physicians and provides needed documentation.
  • Arranges for follow-up care and appointments for patients.
  • Assists with patient triage to assess patient needs and make decisions about appropriate levels of care.
  • Provides education to patients/guardians as needed.
  • Assists with routine office duties to include ordering medical supplies, maintaining appropriate inventory, scheduling patients, answering telephones, keeping environment need and orderly etc.
  • Adheres to standards and guidelines of Joint Commission, Patient Center Medical Home and Meaningful Use requirements.
  • Participates in performance improvement activities in accordance with applicable standards.
  • Performs the duties of Medical Assistant as assigned.
  • Performs other necessary duties as assigned by the Nurse Care Manager to meet the goal of providing quality health care services.
