The qualified Licensed Practical Nurse administers skilled nursing care to patients on an intermittent basis in their place of residence. Provide nursing services, treatments and diagnostic and preventative procedures as assigned. Initiate preventive and rehabilitative nursing procedures as appropriate. Observe signs and symptoms and report to the physician and RN reactions to treatments, including drugs and changes in the patient's physical or emotional condition. Teach and counsel the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home. Maintain accurate and complete records of observations, treatments, and care of patients. What we’re looking for: A passion to serve and help others live their best lives possible. Graduate of an approved school of practical (vocational) nursing. Licensed practical (vocational) nurse in the state of job posting. Minimum of one (1) year as an LVN in the Home Health Care Setting. Must have current BLS Certification.
Responsibilities
The LPN implements nursing care plans and provides treatments and diagnostic procedures for patients in their homes. They are responsible for monitoring patient conditions, reporting changes to physicians, and educating families on care needs.
