Description Fresh Perspective Home Care is seeking a compassionate and organized Licensed Practical Nurse (LPN) to join our team in a hybrid role supporting both field and administrative operations. This position works directly with nursing clients and caregivers by conducting client assessments, developing and updating care plans, providing caregiver training, completing performance evaluations, and ensuring exceptional quality of care. Requirements Active LPN license in good standing Home Care, care coordination, or case management experience preferred Strong communication, organizational, and problem-solving skills Valid driver’s license, car insurance, and reliable transportation Ability to lift, transfer, and assist up to 40 lbs Ability to uphold client confidentiality Ability to manage multiple priorities in a fast-paced environment
