Location: This position is based in Ft Myers, FL, serving patients in Lee County Your Role as a Licensed Practical Nurse (LPN): * Deliver high-quality nursing care to patients in their homes based on individualized care plans. * Perform a variety of tasks, including: * Acute and chronic disease management. * Medication management and administration. * Assessing patient conditions and reporting changes. * Developing tailored, goal-oriented treatment plans. * Post-surgical rehabilitation. * Educating patients and caregivers to support recovery and independence. * Pain management and wound care. Qualifications: * Unencumbered Florida LPN License (required). * Valid driver’s license, auto insurance, and reliable transportation (required). * IV Certification (required). * Experience in acute care or hospital settings (preferred). * Strong assessment and care planning skills.
