Licensed Practical Nurse - Full Time and Part Time openings!   Manage residents existing care plans and supervising the day-to-day resident care activities during the shift. Report any ongoing trends to the Clinical Nurse Manager/Nursing Supervisor. * Complete medical forms, reports, evaluations, studies, charting, care plans etc. as needed. Attends care plan meetings as needed. * Meet with interdisciplinary team members as needed to plan and coordinate shift services and programs. * Admit, transfer, and discharge residents. * Charting and Documentation * Drug Administration * Personnel Functions/Supervision/Leadership * Nursing Care Functions * Monitor seriously ill residents as needed. * Professional Development * Safety and Infection Control * Equipment and Supplies * Resident Rights Supervisory Responsibilities                                                       Directly supervise Certified Nursing Assistants employees in accordance with the organization's policies and all applicable laws. Plan, assign, and direct work, as well as address complaints and resolve problems. Education and/or Experience                                                    * One year certificate from college or technical school for Nursing required. * Minimum one year of long term care or acute care experience required. * Valid NYS LPN license and CPR required.
